Condition observed on or about 05/26/16.  b)  In the area of the CRT-02 Cartoner Machine - Model #3400V - Serial Number E3400V - SA02A000980, 1533 Union Avenue, Pennsauken NJ: Corporate Planned Staffing dba Accu Staffing Services personnel were exposed to electrical hazards when extension cords were used, as they ran under a piece of cardboard on the ground, to power the 3M taping machine, conveyor belt, paragon printer, etc. Condition observed on or about 05/26/16.
