29 CFR 1910.305(g)(1)(iv)(A): Flexible cords were used as a substitute for the fixed wiring of a structure; a) In the area of the CRT-02 Cartoner Machine - Model #3400V - Serial Number E3400V - SA02A000980, 1533 Union Avenue, Pennsauken NJ: J & J Staffing Resources personnel were exposed to electrical hazards when extension cords, strung from a wall to a piece of equipment approximately 68 inches above the ground surface below, were used on a permanent basis to power a printer as well as other equipment. Condition observed on or about 05/26/16. b) In the area of the CRT-02 Cartoner Machine - Model #3400V - Serial Number E3400V - SA02A000980, 1533 Union Avenue, Pennsauken NJ: J & J Staffing Resources personnel were exposed to electrical hazards when extension cords were used, as they ran under a piece of cardboard on the ground, to power the 3M taping machine, conveyor belt, paragon printer, etc. Condition observed on or about 05/26/16.

29 CFR 1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety; a) In the area of the CRT-02 Cartoner Machine - Model #3400V - Serial Number E3400V - SA02A000980, 1533 Union Avenue, Pennsauken NJ: J & J Staffing Resources personnel were exposed to electrical hazards when extension cords, strung from a wall to a piece of equipment approximately 68 inches above the ground surface below, were used on a permanent basis to power a printer as well as other equipment. J & J Staffing Resources did not provide formal instruction, practical training, etc. on electrical safety. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.
