29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge: a) At the 15 Grand Lake Circle jobsite: On August 17, 2016, and times prior thereto, employees were exposed to fall hazards of up to 39 in. to the floor, in that a 10-riser stairway did not include a handrail along one side and a stairrail system along an unprotected, open side of five risers as workers used the stairway for second-floor access to engage in framing and carpentry activities.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) At the 15 Grand Lake Circle jobsite: On August 17, 2016, an employee was exposed to fall hazards of up to 6 ft. to the ground below, in that an 8-ft. stepladder was being used with spreaders closed while leaning on the south side of a new home as employees were engaged in carpentry duties.
