29 CFR 1910.307(c)(2)(i): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:    (a) Starbuds, 9670 Dallas Street Unit E Henderson, CO 80640: On and before August 8, 2016, the employer did not ensure that equipment, wiring methods, and installations of equipment in hazardous (classified) locations were intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. Employees used butane, propane and mixed hydrocarbon gas as a solvent for extraction operations.  Hydrocarbons were released after the completion of an extraction run when the column was opened and the waste product was removed. The paper towel dispenser within the hydrocarbon extraction room was not approved for a Class I Division 2 location. This condition exposed employees to a flash fire hazard due to the potential for an ignition source in a room where flammable gases are present.    (b) Starbuds, 9670 Dallas Street Unit E Henderson, CO 80604: On and before August 8, 2016, the employer did not ensure that equipment, wiring methods, and installations of equipment in hazardous (classified) locations were intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. Employees used butane, propane and mixed hydrocarbon gas as a solvent for extraction operations.  Hydrocarbons were released while filling the OBE extractor. The flashlight used to fill the OBE extractor was not intrinsically safe. This condition exposed employees to a flash fire hazard due to the potential for an ignition source where flammable gases are present.

(a) Starbuds, 9670 Dallas Street Unit E Henderson, CO 80640: On and before August 8, 2016, the employer did not ensure that combustible waste material and residues in a building or unit operating area were kept to a minimum, stored in covered metal receptacles and disposed of daily. Employees used butane, propane and mixed hydrocarbon gas as a solvent for extraction operations.  The extraction waste product was stored in a metal trash bin within the hydrocarbon extraction room.  The waste bin was full of waste product and was not disposed of daily.  Residual hydrocarbons were present in the waste bin.  This condition exposed employees to fire and burn hazards.
