Pennington Gap, VA 24277; Instance #1: Date of Violation: On or about 08-17-2016 and times prior thereto, located in the kitchen/galley food serving area, employees were exposed to fire and electrical hazards due to no strain relief on the electrical power cord servicing the food warmer. Instance #2: Date of Violation: On or about 08-17-2016 and times prior thereto, located in the Educational & Vocational Trade Shop, employees were exposed to fire and electrical hazards due to no strain relief on the electrical power cord servicing the Dayton model Commercial Air Circulator Fan. Instance #3: Date of Violation: On or about 08-18-2016 and times prior thereto, located in the Chiller Room/Boiler Shop, employees were exposed to fire and electrical hazards due to no strain relief on the electrical power cord servicing the Dayton model 3C673A 48 inch Commercial Air Circulator Fan. Instance #4: Date of Violation: On or about 08-18-2016 and times prior thereto, located in the Welding Shop, employees were exposed to fire and electrical hazards due to no strain relief on the electrical power cord located at the disconnect box servicing the metal cut-off saw . Instance #2: Date of Violation: On or about 08-18-2016 and times prior thereto, located in the "Power House" Boiler space, employee(s) were exposed to electrical hazards from a damaged TPI Industrial brand electric fan with a broken and missing on/off switch. Employee(s) energized and de-energized fan by plugging and unplugging from electrical power source. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
