29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: Warehouse Areas of 3358 and 3375 Buildings - On or about August 12 to August 17, 2016, employees added water to the batteries of electric powered industrial trucks and were exposed to corrosive chemical splashes to the eyes from the lead acid in the batteries. The employer did not have an eyewash station available in the area for quick drenching or flushing of the eyes. Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1910.178(o)(5): A load engaging means was not placed under the load as far as possible; the mast was not carefully tilted backward to stabilize the load. Warehouse Area, 3375 Building - On or about August 12, 2016, employee(s) used an electric walk-behind powered industrial truck (Clarklift Powrworker Model ST30B) to lift a load of 6 individually boxed and palletized air conditioning units. The units each weighed approximately 120 lbs. and were shrink-wrapped together in pairs. The forks of the lift truck were not placed under the load as far as possible, resulting in the load becoming unstable and falling from the forks. Employees were exposed to struck-by and crush hazards from the falling load. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
