29 CFR 1910.178(o)(5): A load engaging means was not placed under the load as far as possible; the mast was not carefully tilted backward to stabilize the load.   Warehouse Area, 3375 Building - On or about August 12, 2016, employee(s) used an electric walk-behind powered industrial truck (Clarklift Powrworker Model ST30B) to lift a load of 6 individually boxed and palletized air conditioning units.  The units each weighed approximately 120 lbs. and were shrink-wrapped together in pairs.  The forks of the lift truck were not placed under the load as far as possible, resulting in the load becoming unstable and falling from the forks.  Employees were exposed to struck-by and crush hazards from the falling load.       Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
