29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: On or about; July 28, 2016 Location: 2848 Bainbridge Avenue Bronx, NY 10458, Residential 4 story building, 1st floor, Rooms # 105, 106 and 107 a) Employees engaged in residential building renovation activities at the first floor level were not protected from trip/fall hazards by keeping all work areas clear from scrap lumber with protruding nails, and all other debris.

The path to ground from circuits, equipment, and enclosures was not permanent and continuous: On or about; July 28, 2016 Location: 2848 Bainbridge Avenue Bronx, NY 10458, Residential 4 story building, 1st floor, Rooms # 105, 106 and 107 a) Employees engaged in residential building renovation activities at the first floor level were exposed to electrical hazards by using an ungrounded extension cord (broken ground prong on the cord plug) to energize a Bosch P6325 saw.
