29 CFR 1910.22(b)(1): Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repairs, with no obstruction across or in aisles that could create a hazard. On August 11, 2016, the aisle and pedestrian passageway was not kept clear due to two 4' x 4' x 6' product pallets blocking the pedestrian passageway forcing a pedestrian to walk out into the aisle and was struck by a forklift operator.

29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing. On August 11, 2016 in the aisle way just past the "tunnel" from Warehouse #2, a pedestrian was exposed to a struck-by hazard due to two pallets, 4 ft. x 4 ft. x 6 ft. blocking the pedestrian walkway and creating an obstructed view of the aisle way on the right and the forklift operator did not slow down and sound the horn for pedestrians having to walk around the pallets.
