LMN Development, LLC located in Sandusky, Ohio:  On or about July 2, 2016, the employer did not ensure employees performing electrical work and exposed to electrical hazards were properly trained on the following topics:        1. How to properly lockout electrical systems and to verify the deenergization of electrical systems;    2. How to inspect electrical protective equipment;    3. What personal protective equipment needs to be worn when performing electrical work;    4. Proper use and inspection of electrical testing equipment;    5. Electrical shock and arc blast/flash hazards;

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were  or could have been energized. The specific safety-related work practices were not consistent with the nature and extent of the associated electrical hazards.    a. LMN Development, LLC located in Sandusky, Ohio:  On or about July 2, 2016, the employer did not ensure an employee rewiring a 110 volt light switch, deenergized the electrical energy to the switch prior to performing work.  The employee received a severe electrical shock which required him to seek medical treatment.    b. LMN Development, LLC located in Sandusky, Ohio:  On or about August 15, 2016, the employer did not ensure an employee working inside of the 480 volt electrical panel SP-3B in Building B North, deenergized the electrical panel prior to performing work inside of the panel.  The employee received a severe electrical shock which required him to seek medical treatment.

29 CFR 1910.333(b)(2)(i): The employer did not maintain a written copy of the procedures outlined in paragraph (b)(2) and shall make it available for inspection by employees and by the Assistant Secretary of Labor and his or her authorized representatives.    a. LMN Development, LLC located in Sandusky, Ohio:  On or about July 2, 2016, the employer did not ensure electrical system lockout procedures were developed which covered operations such as but not limited to employees working inside of the 480 volt electrical panels to troubleshoot fuses and employees rewiring new light switches inside of guest rooms.

Interlocks for electric equipment may not be used as a substitute for lockout and tagging procedures.    a. 29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards were not provided with, and did not use, electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed.     a. LMN Development, LLC located in Sandusky, Ohio:  On or about July 2, 2016, the employer did not ensure employees performing electrical work and exposed to electrical hazards used protective equipment for the hands when rewiring a 110 volt light switch.     b. LMN Development, LLC located in Sandusky, Ohio:  On or about August 15, 2016, the employer did not ensure employees performing electrical work and exposed to electrical hazards used protective clothing for the upper body, lower body, face, head, and hands when troubleshooting a 480 volt fuses inside of electrical panel SP-3B located in Building B North.
