29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about August 15 - 16, 2016, at 2750 Blackmon Drive, Decatur, Georgia: An employee performing stucco work was exposed to a fall hazard of approximately 33-feet when he climbed the frame of a tubular welded frame scaffold.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about August 15, 2016, at 2750 Blackmon Drive, Decatur, Georgia: Two employees performing stucco work were exposed to a fall hazard of approximately 58.6 feet when they worked without attaching their life lines to the aerial lift baskets they worked from. Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated
