29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: a) Worksite located at 7613-7619 NW Prairie View, Platte Woods, MO - Employees were exposed to a fall hazard of a distance of approximately 14 feet 8 inches, in that defective components of a Miller Ultra personal fall arrest system were not removed from service. Straps on the harness were visibly stretched and deformed.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Worksite located at 7613-7619 NW Prairie View, Platte Woods, Missouri - Employees were exposed to a fall hazard of a distance of approximately 14 feet 8 inches, in that employees were working on an unprotected edge without any type of fall protection system in use. Fevco dba Classic Roofing was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 393225, citation number 1, item number 1 and was affirmed as a final order on May 16, 2012, with respect to a workplace located at100 E. Washington Street, Kearney, Missouri.
