29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:     (a) On and before August 18, 2016, employees were exposed to serious injuries from a fall hazard, in that the NW Corner Exit Door in the warehouse area had a 19-1/2 foot drop from the floor to the ground. As result, employees were exposed to a trip and fall hazard in the event of an evacuation from the door.       Abatement certification for this violation is not required.

29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident:         (a) On August 18, 2016, the employer had not ensured that an operator involved in a near-miss struck-by incident on May 31, 2016, had been given Refresher Training to include an re-evaluation of that training. The operator struck a pedestrian who received serious injuries to his foot requiring medical attention.        Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
