(b) Location - 83 Turnpike Road, Ipswich, MA: On or about 8/18/2016, approval from a recognized authority was not secured prior to the use of a work platform attachment to elevate personnel with a forklift, and capacity, operation, and maintenance instruction plates were not changed accordingly.
