29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    On or about 18 August 2016, the employer did not establish an energy control (lockout/tagout) program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee were required to gain access to machines, including the CNC's, Laser cutters, CNC Press brakes, and Table sanders, for tool changes, servicing and maintenance, where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine would be isolated, and rendered inoperative. Employees were exposed to caught-in hazards likely to result in crushing injuries and/or amputation from unexpected energizing, start up or release of stored energy.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    On or about 18 August 2016, the employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. Employees were exposed to injury form struck-by hazards.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.     On or about 18 August 2016, the employer failed to guard the point(s) of operation of machinery to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s) on the following machines:    a. Amada Fox B III-1253 S/N 12530025- The point of operation of the press was not guarded to prevent employees from gaining access during operation. This exposed employees to a crushing hazard.    b. Amada RG-80 S/N 811309- The point of operation of the press was not guarded to prevent employees from gaining access during operation. This exposed employees to a crushing hazard.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:    On or about 18 August 2016, the employer did not develop, implement, and maintain a written hazard communication program at the workplace which described how the criteria specified in 1910.1200(f), (g), and (h) will be met for employees exposed to hazardous chemicals, including cutting fluids, oils, propane, paint thinner, and cleaning chemicals.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
