29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) On or about August 16, 2016, a temporary employee whose day-to-day activities were directed by Brinkmann Constructors, was injured and required inpatient hospitalization. Brinkmann Constructors did not report the incident to OSHA. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
