29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501. On or about August 15 - 16, 2016, at 2750 Blackmon Drive, Decatur, Georgia: a) Three employees performing framing work on Building Phase 4 were exposed to a fall hazard of approximately 11.8-feet when they worked at the unprotected edges of a Building without using fall protection. b) An employee performing framing work on Building Phase 3 were exposed to a fall hazard of approximately 11.8-feet when he worked within 6 feet of the unprotected edges and near a floor opening without attaching to his life line. c) Two employees performing framing work on Building Phase 1 were exposed to a fall hazard of approximately 48.3-feet when they worked at the unprotected edges of a Building without using fall protection.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about August 15, 2016, at 2750 Blackmon Drive, Decatur, Georgia: An employee performing framing work was exposed to a fall hazard of approximately 11.8-feet when he worked with an attached lanyard line was 22 feet too long and it would have allowed him to strike the ground.
