29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this sec  1) Production Area: On or about August 18, 2016, an employee right index finger was amputated while cleaning the rollers.  The employer failed to implement written lockout/tagout procedures on all roll mill machines for employees conducting service and maintenance.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  1) On or about August 18, 2016, the employer failed to report the work-related amputation that occurred on August 11, 2016 to the Atlanta West Area Office of the Occupational Safety and Health Administration.
