29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:    Worksite located at 975 N Mustang Rd. Tuttle, OK 73089. On or about August 17, 2016 employees were exposed to a fall hazard of 15 feet while working on fabricated frame scaffolding that was not provided with cross braces to connect the frames and panels.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Worksite located at 975 N Mustang Rd. Tuttle, OK 73089. On or about August 17, 2016 employees did not wear protective head gear while performing masonry work, exposing employees to hazard of being stuck-by falling objects.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    Worksite located at 975 N Mustang Rd. Tuttle, OK 73089. On or about August 17, 2016 employees were exposed to a fall of greater than 10 feet while performing masonry work from scaffolding that had missing guardrails and no other forms of fall protection.       CIA Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 986395, citation number 01, item number 01 and was affirmed as a final order on 09/26/2014, with respect to a workplace located at 2500 Conference Drive, Norman, OK 73069.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    Worksite located at 975 N Mustang Rd. Tuttle, OK 73089. On or about August 17, 2016 employees were observed working from scaffolding that was not provided with protection from falling objects through the use of toeboards, screens, or other protective measures. The employees were exposed to struck-by hazards including, but not limited to, bricks or hand-tools.      CIA Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(h)(1), which was contained in OSHA inspection number 985623, citation number 01, item number 01b and was affirmed as a final order on 09/26/2014, with respect to a workplace located at 2500 Conference Drive, Norman, OK 73069.
