29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a. On or about August 16, 2016, at the above addressed jobsite, an employee conducting tuck point operations from a tubular welded frame scaffold, eighteen (18) feet nine (9) inches above the ground, was not provided with an access ladder, thereby exposing the employee to falls.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a. On or about August 16, 2016, at the above addressed jobsite, employees conducting masonry operations from a tubular welded framed scaffold, eighteen (18) feet nine (9) inches above the ground, were exposed to fall hazards due to the lack of a complete guardrail system.
