OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards while using a Rotary brand automotive lift that was exhibiting signs of erratic operation. a) Garage area, Bay Six, 500 Chauvet Drive, Pittsburgh, PA 15275: On or about June 23, 2016, a Rotary brand four post automotive lift (model number SM18N010RD) that was exhibiting signs of erratic operation was not immediately removed from service. A lock that was not engaging caused vehicles to tilt while on the lift. Employees who continued to use the lift until it was repaired were exposed to struck-by and crushed-by hazards.

Employees were exposed to fire hazards created by the obstructed exit route.
