29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    On or about August 17, 2016, at the above addressed job site, the employer did not ensure that frequent and regular inspections of the job site were made around the truck, to ensure that the scaffold suspension cables were not subject to damage or disturbance and were not in the path of the wheels of the truck.

29 CFR 1926.451(d)(3)(x): Tiebacks were not installed perpendicular to the face of the building or structure, nor were opposing angle tiebacks installed:    On or about August 17, 2016, at the above addressed job site, employees who had been performing concrete restoration work from the East suspended swing stage scaffold, were exposed to fall hazards as the scaffold had been secured from 2 parapet clamps, each with a single tie back that had not been installed perpendicular to the face of the building.

29 CFR 1926.451(g)(1)(ii): Each employee on a single-point or two-point adjustable suspension scaffold was not protected by both a personal fall arrest system and a guardrail system:    On or about August 17, 2016, at the above addresses job site, an employee who was performing masonry work from a swing stage scaffold without a means of fall protection was exposed to a 60-foot fall hazard.

29 CFR 1926.451(g)(3)(i): Vertical lifelines were not fastened to a fixed safe point of anchorage, independent of the scaffold, and protected from sharp edges and abrasion:    On or about August 17, 2016, at the above addresses job site, employees who were performing concrete restoration work from swing stage scaffolds had been using lifelines that were not in conformance with this standard:    2 Instances  a. Five lifelines lacked abrasion protection that were in direct contact with sharp edges of the roof and the parapet wall.  b. Two lifelines were secured to a ladder and lifelines were secured to parapet clamps that supported  tiebacks.

29 CFR 1926.451(d)(12)(iv): Wire rope clips were not inspected and retightened to the manufacturer's recommendations at the start of each work shift and thereafter:    On or about August 17, 2016, at the above addressed job site, employees who had been performing concrete restoration work, were exposed to a fall hazard while using swing stage scaffolds which were suspended by wire rope cables from parapet clamps which had tie backs that that had wire rope clamps that had not been retightened to the manufacturer's recommendations at the start of each work shift.
