29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 8/11/2016, in the Grinding Room, after a planned power outage employee did not utilize lockout procedures when troubleshooting FG35 grinder when it wasn't turning on. Exposing employee to amputation hazards. b) On or about 8/18/2016, in the Grinding Room, when employees perform maintenance and servicing activities, such as cleaning the grinder, the shift change energy control procedures were not being utilized. Exposing employee to amputation hazards ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. a) On or about 08/18/16, in the Grinding Room, employer did not provide training to authorized employees such as machine operators who are required to Lockout machines such as, but not limited to, the FG35 grinder to unjam or clean the product out. ABATEMENT DOCUMENTATION REQUIRED
