Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a. On or about August 19, 2016, at the construction site located at 264 South Hollywood Blvd., Steubenville, Ohio, employees were performing commercial roofing work on a steep roof without fall protection, therefore exposing employees to a fall hazard of approximately 25 feet.

29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines. a. On or about August 19, 2016, at the construction site located at 264 South Hollywood Blvd., Steubenville, Ohio, employees used an extension ladder to access a flat roof that had warning lines erected. The ladder point of access was not connected to the work area with two warning lines as an access path, therefore allowing employees between the warning line system and roof edge without fall protection, exposing employees to a fall hazard of approximately 25 feet.
