29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) On or about August 20, 2016, Bob Floor's Roofing, Inc., as the controlling employer, did not ensure that employees of AC Exteriors, LLC., a subcontractor, were protected from falls while working at heights greater than 6 feet. Employees of AC Exteriors, LLC. were exposed to fall hazards in excess of 23 feet while engaged in roofing activities on a two- story residential building with no fall protection in place. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: (a) On or about August 20, 2016, Bob Floor's Roofing, Inc., as the controlling employer, did not ensure that portable ladders, used to access upper landing surfaces, extend at least 3 feet above the upper landing surface to which the ladders were used to gain access. Employees of AC Exteriors, LLC., a subcontractor, were observed using a ladder to access the roof of a residential building. Ladder did not extend at least 3 ft above the landing platform exposing employees to falls of up to 8 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
