Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    On or about August 13, 2016 at approximately 4:30am at or near 4320 E. McCain Blvd, North Little Rock, AR 72117, an employee was hospitalized after being shot by an assailant in the shoulder and face. The in-hospitalization was reported on August 14, 2016 at about 10:00am, which was not within the stipulated 24 hours timeframe.
