29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  A. Adam Elliott Construction Company LLC worksite located at 2864 Longview Dr., Maumee, Ohio: On or about July 12, 2016, the employer did not ensure employees were wearing proper eye protection while operating pneumatic nail guns when installing weatherproofing on the East die of the two story residential structure. The employees were exposed to potential flying object hazards.   Adam Elliott Construction Company LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.95(a), which was contained in OSHA inspection number 977963, citation number 1, item number 1 and was affirmed as a final order on July 9, 2014 with respect to a workplace located at Perrysburg, Ohio.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    A. Adam Elliott Construction Company LLC worksite located at 2864 Longview Dr., Maumee, Ohio: On or about July 12, 2016, the employer did not ensure employees were protected from falls while installing roofing materials on the two story residential structure. Employees were exposed to a fall distance of approximately 18.00 feet.   Adam Elliott Construction Company LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 988890, citation number 1, item number 1 and was affirmed as a final order on November 18, 2014, with respect to a workplace located at Whitehouse, Ohio.
