29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section. On or about August 12, 2016, at the Sunoco Logistics Plant in Nederland, Texas; the lead technician failed to follow specific procedural steps for isolating, blocking, and securing machines or equipment to control hazardous energy; The following sections of the employers' multi Bolt Hydrostatic Isolation Field Procedure were not implemented: Section 2.10 requires, -Install the vent pipe, fitting, 0-15 psi vent gauge and vent hose. And Section 2.14 which states: a. While the tool is installed, at no time shall there be any pressure behind the tool. b. Install the vent pipe, fitting, 0-15 psi vent gauge and vent hose c. While the tool is installed, at no time can parts (vent gages or hoses) be removed. This practice is strictly forbidden. When the venting assembly is removed from the isolation tool, the isolation is no longer providing the required the required vapor barrier.
