29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:     (a) Worksite - Street-side of the residential structure:  On or about 8/22/2016, the employer did not ensure that overhead power lines were effectively guarded to protect employees from incidental contact while setting-up a forty foot aluminum extension ladder in the immediate area.

29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have non-conductive side-rails:      (a) Worksite - Street-side of residential structure:  On or about 8/22/2016, the employer did not ensure that a non-conductive extension ladder was used at the street-side of a residential structure, where it could come in contact with overhead power lines.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:    (a) Worksite:  On or about 8/22/2016, the employer did not ensure that employees were properly trained to recognize the hazards associated with ladder set-up and use, specifically as it pertains to using aluminum extension ladders in near proximity to overhead power lines.
