29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). 117 Conz Street, Northampton, Massachusetts; On August 22, 2016 an employee was exposed to cave-in hazards in a trench that was 5.8 feet deep. The trench was in a roadway and was not protected against cave-ins by a trench box, scaling, or use of benching.
