29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section: Well site: On or about August 19, 2016, and times prior to, employees engaged in the installation of production piping of an oil/gas well were provide an angle grinder. The grinder was not equipped with a guard exposing the employees to the hazard of being struck by fragments from the disintegrating abrasive wheel.

No ground fault protection was provided exposing the employees to the hazard of electrical shock.
