29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: W.G. Hefner VA Medical Center: On or about 08/18/2016 an employee was using an extension ladder improperly. The ladder was not properly secured.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: W.G. Hefner VA Medical Center: On or about 08/18/2016 an employee was on a 21 foot extension ladder with his body turned away from the ladder rungs and a bucket in one hand. 3-point contact was not maintained while applying hydraulic cement onto a pre-assembled modular wall.
