29 CFR 1926.753(c)(2): A qualified rigger did not inspect the rigging prior to each shift in accordance with 29 CFR 1926.251: Well site: On or about August 19, 2016, and times prior to, employees were using a hydraulic crane to install a pumping unit on an oil/gas well. The rigger was not qualified exposing the employees to the hazard of being struck by a falling load.

29 CFR 1926.1428(a): The employer of the signal person did not ensure that each signal person met the Qualifications Requirements (paragraph (c) of this section) prior to giving any signals: Well site: On or about August 19, 2016, and times prior to, employees were assembling and installing a pumping unit on an oil/gas well using a crane. The signal person did not meet the qualifications requirements exposing the employees to the hazard of being struck by a suspended load.
