OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees On or about August 17, 2016, at the Mitek Cybersaw A/T, the air brake for the saw blade was not working. The saw blade kept rotating once the emergency stop was activated. Employees exposed to an amputation hazard on this particular saw.

29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requires of 1910.219: (a) On or about August 17, 2016, at the web saw, there were several rotating shafts that protruded out more than one half the diameter. Employees exposed to the hazard of being caught-in the rotating shaft. Reference 1910.219(c)(4)(i). (b) On or about August 17, 2016, at the web saw, the keyway shaft was protruding out and was not guarded. Employees exposed to the hazard of being caught-in the rotating keyway shaft. Reference 1910.219(c)(4)(ii). (c) On or about August 17, 2016, at the web saw, the chain and sprockets were not guarded. Employees exposed to the hazard of being caught-in the rotating chain and sprocket. Reference 1910.219 (f)(3). (d) On or about August 17, 2016, at the blade saw, the chain and sprocket were not guarded. Employees exposed to the hazard of being caught-in the rotating chain and sprocket. 29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material being cut: On or about August 17, 2016, at the Speed Cut radial arm saw, the sides of the blade were not guarded to the full diameter of the blade by a device that adjust itself to the thickness of the material being cut. The employee was cutting wood and was exposed to an amputation hazard from the rotating blade. The manufacture of the saw was Speed Cut Inc, model SSA-17.
