29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) (a) location; Hollis Woods Lot #6, Holliston, MA. On or about August 22, 2016, the employer did not conduct frequent or regular inspection of a WERNER pump jack scaffold prior to use. The left pump jack pole top bracket failed causing an employee to fall from a height greater than 15 feet above lower and suffered severe injuries.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. (a) location; Hollis Woods Lot #6, Holliston, MA. On or about August 20-22, 2016, a WERNER pump jack scaffold was not secured at the bottom with a rigid triangular bracing or equivalent on each scaffold pole. This was a contributing factor when a top triangular bracket failed causing an employee to fall to greater than 15 feet to lower level and suffer severe injuries.
