29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    Employees were exposed to burns and struck by hazards while removing jams on an Impulse-1 machine, and the procedures did not clearly and specifically outline the techniques to be utilized regarding the following:     -specific methods to be used for the placement, removal, and transfer of lockout and/or tag out devices,    -specific requirements for testing the machine to ensure the effectiveness of the lock out, tag out, or other energy control measures.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tag out procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about July 16, 2016, an employee's fingers were exposed to electrical burns and being pinned under the cutting arm of an Impulse-1 Machine, during servicing and removing jams on the bag feeding process.   The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(I)], and verification of isolation [(d)(6)], were not implemented to protect employees form machine servicing hazards.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    An employee was exposed to nip point hazards while attempting to remove a bag from an Impulse Machine, and the employer did not adequately guard the hazard with a light curtain.   The light curtain was set approximately 2 1/2 inches from the point of operation, and not at the manufacturer's required distance (that is at least greater than 4 inches).

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7)  feet or less above floors or platforms were not enclosed:    An operator was exposed to approximately 1/4-inch metal sprocket wheels and chains which were located under an approximately 3-foot high work table for the Impulse machines.  The sprocket wheels and chains were not enclosed or guarded.
