29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section: While setting shims on a manual header, the employees hands and fingers were exposed to amputation hazards due to the employer's energy control procedure for the manual header did not clearly and specifically outline the following: -The specific steps for shutting down the machine noted a push red mushroom stop button on the operator's control panel. No red mushroom stop button was present on the panel box displayed in the procedure. -No specific steps were provided to verify the isolation of hazardous energy.
