29 CFR 1926.20(b)(2): Frequent and regular inspections of the job sites, materials, and equipment were not made by a competent person(s) designated by the employer:  (a) Jobsite, 773 county route 9A:  On or prior to 8/23/16, jobsite was not inspected by a competent person who was familiar with the hazards and applicable standards which applied to poured in-place concrete work, including, but not limited to:  provision of first aid, selection and use of personal protective equipment, protection of exposed rebars, safe use of ladders, safe use of concrete formwork, and safe use of scaffolding.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  (a) Jobsite, 773 county route 9A:  On or prior to 8/23/16, the employer did not ensure that employees wore steel toe boots or equivalent protective footwear, safety glasses, and hard hats while building concrete formwork and rebar assemblies.
