29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    The employer is failing to protect employees from fall hazards. This was most recently documented at 1315 Hearne Circle Raymore, Missouri 64083. Employees were observed approximately 15 feet up working on removing roofing tiles from a 2nd story house with a roof pitch ranging from of 6/12 to 12/12.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   The employer is failing to protect employees from serious struck by hazards. This was most recently documented at 1315 Hearne Circle Raymore, Missouri 64083. Employees were observed removing roofing tiles with pitch forks without the use of eye protection.  Servando Gomez, LLC was previously cited a violation of an equivalent Occupational Safety and Health Standard 1926.102(a)(1), which was contained in OSHA inspection number 1082290, citation number 1 item number 1 and was affirmed as a final order on August 26, 2016, with respect to a workplace located at 8401 84th Street Prairie Village, KS 66208.
