On August 16, 2016 located at the Us Bank Tower 425 Walnut St. Cincinnati, OH employees    involved in sign associated removal and replacement activities were utilizing a two point adjustable suspension scaffolding comprised of an Altrex modular platform with Power Climber PC1 Series hoists SN. L99B7654 & SN. D06B2183 components on which the Power Climber PC1 Series hoists and vertical lifeline scaffold and scaffold components were not inspected / tested for defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.

On August 16, 2016 located at the Us Bank Tower 425 Walnut St. Cincinnati, OH employees    involved in sign associated removal and replacement activities were utilizing a two point adjustable suspension scaffolding on which employees suspended personal fall arrest vertical lifeline systems were in contact with a parapet wall edge and not protected from abrasion or softened exposing employees to fall potentials in excess of 100 feet.
