29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a. At 715 East Fayette Street, Syracuse, NY 13204, on or about 8/19/16: Employees performing roofing work on a flat roof less than fifty feet in width. Portions of the roof edge were protected by a parapet which ranged between eighteen and thirty-nine inches. Other portions of the roof were not protected at all. Employees were not wearing personal fall arrest systems and no other type fall protection was used.

At 715 West Fayette Street, Syracuse, NY 13204, on or about 8/19/16: Employees accessing the upper level of a commercial building. A wooden "trestle style" ladder was used to access the roof. The side rails did not extend beyond the upper landing surface.
