29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them.  a. In the southwest production bay, an employee removing a sheet metal wall contacted an energized 240 volt buss bar with his head, causing the employee to go into cardiac arrest, on or about 8/17/2016.   Or in the alternative:   29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means.  a. In the southwest production bay, an employee removing a sheet metal wall contacted an energized 240 volt buss bar with his head, causing the employee to go into cardiac arrest, on or about 8/17/2016.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.  a. An employee suffered electrical shock when his head contacted a 240 volt buss bar with his head. He went into cardiac arrest and was hospitalized on 8/17/2016. The employer did not report the hospitalization to OSHA until 8/22/2016.
