John Scott DBA Old Oxford General Contractors worksite located at 4959 Black Rd., Monclova, Ohio: On or about August 23, 2016, the employer did not ensure an employee was protected from fall hazards while installing siding on the south side of the two story structure. An employee was exposed to a fall distance of approximately 12.00 feet while working on the pump jack scaffold system.

John Scott DBA Old Oxford General Contractors worksite located at 4959 Black Rd., Monclova, Ohio: On or about august 23, 2016, the employer did not ensure the top of an 8 ft A-framed stepladder was not use as a step by an employee. The employee was using the top of the ladder to access the gable on the south side of the two story residential structure. The employee was exposed to fall hazards due to improper ladder use of approximately 18.00 feet.
