29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: (a) At the intersection of Cypress Ct and Maple St, Menomonie WI.; Two worn extension cords provided with power, utilized by employees, were not free of physical damage such as cuts. i) The yellow extension cord utilized to provide power to cutting and drilling tools had a cut insulating sheath and ii) The orange extension cord utilized to provided power to cutting and drilling tools had a cut insulating sheath.
