29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form. a) Elyria Anesthesia Services Inc. located at 860 East broad St. Ste. 1 Elyria, OH, 44035: On or about October 18, 2016 when employees, working at Mercy Allen Hospital located at 200 West Lorain St. Oberlin , OH, 44074, were exposed to anesthetic gasses during medical procedures the employer did not conduct a reasonable estimate of employee exposures to respiratory hazards. Not conducting an reasonable estimate of employee exposure to respiratory hazards exposes employees to a potential overexposure to anesthetic gasses.
