29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:        a) On or about August  23, 2016, employees were exposed to the hazard of falling up to approximately 16 feet while climbing portable extension ladders that did not extend above the landing surface a roof deck.        No abatement certification or documentation required for this violation.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):        a) On or about August 23, 2016, employees were exposed to the hazard of falling approximately 16 feet while working from the roof of a residential structure without fall protection.       Redhawk Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1019933, citation 2, item 1, and was affirmed as a final order on February 20, 2015, with respect to a worksite located at 20585 W. Highridge Drive, Kildeer, Illinois.    Redhawk Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 959134, citation 1, item 2, and was affirmed as a final order on May 16, 2014, with respect to a worksite located at 22117 Windridge Ct., Kildeer, Illinois.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
