29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. Site: 39 East 72nd Street New York NY On or about 8/24/16 a) Employees were exposed to fall hazards from the 4th level to the 3rd level due to unguarded holes. The employees were not using fall protection. The Alba Services Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1) which was contained in OSHA inspection numbe1118429, citation number 1, item number 2 and was affirmed as a final order on 4/26/2016, with respect to a workplace located at 436 Keap Street Brooklyn, NY.
