29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: On or about August 23, 2016, and times prior thereto, in the shop on the left side of the shop entrance door, employees operated switchboard panels which had breaker slots missing filler plates, such as, but not limited to the: a) 120-208 Volt panel; and, b) 120-240 Volt panel.
