29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    On or about 24 August 2016, the Employers written lockout/tagout program did not include procedures that clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of unexpected energizing, start up or release of stored energy. Employees gaining access to the machines such as shears, brake presses, laser cutting machines and lathes, who perform servicing, maintenance, and tool changes were exposed to caught in hazards.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    On or about 24 August 2016, the employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed for the following types of machines: shears, brake presses, laser cutting machines and lathes.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    On or about 24 August 2016, the employer failed to provide machine guarding to protect operator(s) and other employees from hazard(s) created by the following machines:    a. 357 Anoca. The machine did not have a guard on the rear part of the machine. This allowed the employees to gain access when they would load sheet metal into the rear of the machine.     b. SM-171 Bystronic Suction Cup Feeder. Machine is an automatic fed machine and there is no guarding to prevent employees from gaining access to the moving parts of the machine. This exposed employees to a struck by and caught in hazard.    c. SM-191 Bystronic Auto Feeder. Machine is an automatic fed machine and there is no guarding to prevent employees from gaining access to the moving parts of the machine. This exposed employees to a struck by and caught in hazard.    d. Amada Pega SM-231. Machine is an automatic fed machine and there is no guarding to prevent employees from gaining access to the moving parts of the machine. This exposed employees to a struck by and caught in hazard.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    The employer failed to guard the point(s) of operation of machinery to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s) on the following machines:    a. SM-141 Amada Shear, SN 30600672- The guarding on the shear was not adequately adjust to within 1/4in to prevent employees from gaining access to the point of danger when the shear was in operation. This exposed employees to a crushing and shearing hazard.    b. SM-187, SM-184, SM-213 and SM-220 Cincinnati Press Brake- The point of operation of the press was not guarded to prevent employees from gaining access during operation. This exposed employees to a crushing hazard.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.    On or about 24 August 2016, the employer failed to install and use equipment in accordance with the instructions listed and permitted uses. A knockout type box was installed as a pendant drop cord and used in the production area. Employees were exposed to a shock hazard.    Abatement documentation is not required for this item.
