29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.  On or about 23 August 2016, and at times prior thereto, an Acetylene cylinder was sitting on the floor of the mechanic shop in close proximity to 5 Oxygen cylinders without a fire-resistance barrier or a distance of at least 20 foot separation, exposing employees in the shop to the hazard of a fire, explosion, or being struck by flying debris and projectiles.

29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders were not located where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons.  On or about 23 August 2016, and at times prior thereto, an Acetylene cylinder was sitting on the floor within the mechanic shop without being secured, while oxygen cylinders were stored within 7 feet away, exposing employees in the shop to a struck by hazard from the cylinder falling over.

On or about 23 August 2016, and at times prior thereto, employees used two outlet receptacles located above a sink and within the kitchen/break room that were not ground-fault circuit-interrupter protected, exposing the employees to an electrical hazard of approximately 120 volts.
