29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    On or about August 24, 2016 and times prior thereto, at the worksite located at or near 3904-B Kelley Avenue in Springdale, Arkansas, the employer had not developed or implemented a respiratory protection program for employees required to use N-95 dust masks. This condition exposed employees to inhalation hazards.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    On or about August 24, 2016 and times prior thereto, at the worksite located at or near 3904-B Kelley Avenue in Springdale, Arkansas, the employer had not developed a hazard communication program for employees exposed to, but not limited to silica, a two-part epoxy, acetone, super glue, and wood dust. This exposed employees to chemical hazards.
